We require the following information for an instant indication:
- Applicant Name
- Address including Zip code of property to be insured
- Value of building (total value of main building excluding other structures)
- Liability? Y/N If yes, advise limit required ($100k, $500k, $1m)
- Term: 3, 6, 9 or 12 months
IF Commercial Property:
- Coverage: Basic or Special Cause of Loss
- Construction
- Protection Class
- Total Square Footage of building to be insured including outbuildings
- Age of building or date of complete building upgrade
- Number of Stories
- Vandalism coverage required? Y/N
- Are there any Other Structures to be insured?
- AOP Deductible per occurrence
- Is TRIA Coverage required? Y/N
- Is the property to be insured undergoing any renovation or construction work of any kind, or is any such work due to commence while insurance is in effect?
IF Residential Property:
- Construction
- Coverage Form Desired: DP1 or DP3
- Protection Class
- Age of building or date of complete building upgrade
- Vandalism coverage required? Y/N
- Are there any Other Structures to be insured?

For a FIRM QUOTE in addition to the information requested above provide answers to the additional questions:
- Period the property has been vacant?
- Is the building(s) to be insured secured against unauthorized entry?
- Has the applicant had any policy of property insurance cancelled or non-renewed in the past 3 years for reasons other than vacancy?
- Is the applicant currently involved in bankruptcy proceedings
- Is the property to be insured subject to mortgage foreclosure proceedings or tax liens?
- Has the property to be insured been condemned or is it scheduled for demolition?
- Existing damage to building(s) to be insured
- Is the property to be insured subject to more than two mortgages or other encumbrances?
- Has the applicant been convicted of the crimes of arson or insurance fraud?
- Is the property to be insured undergoing any renovation or construction work of any kind, or is any such work due to commence while insurance is in effect?
Miscellaneous:
- If the risk falls into PC 9 or 10, the max sum insured is reduced to no greater than $350,000
- No swimming pool liability coverage is available under this program
- Insurance is only available for properties that are 100% vacant
